FMCSA Drug & Alcohol Clearinghouse — Limited Query Consent

I provide consent to Indiana Dynamic Logistics LLC to conduct a limited query of the FMCSA Commercial Driver's License Drug and Alcohol Clearinghouse to determine whether drug or alcohol violation information about me exists in the Clearinghouse. I understand that if the limited query shows information exists, Indiana Dynamic Logistics LLC must obtain additional specific consent before requesting a full query.

I understand that if I refuse to provide consent for a limited query, Indiana Dynamic Logistics LLC must prohibit me from performing safety-sensitive functions, including driving a commercial motor vehicle, as required by FMCSA regulations.

I have read and consent to the FMCSA Clearinghouse limited query described above.

Motor Vehicle Record / Driving Record Authorization

I authorize Indiana Dynamic Logistics LLC and its agents to obtain and review my motor vehicle records and driving history for employment-related purposes as permitted by applicable law and FMCSA regulations.

FMCSA Pre-Employment Screening Program (PSP) Disclosure & Authorization

Indiana Dynamic Logistics LLC may obtain background reports containing crash and inspection history from the FMCSA Pre-Employment Screening Program for employment purposes. The information may include data from the Motor Carrier Management Information System, including crash and inspection information.

I authorize Indiana Dynamic Logistics LLC to access the FMCSA PSP to seek information regarding my commercial driving safety record and information regarding my safety inspection history. I understand that I may obtain a copy of the report and may challenge inaccurate information through the applicable FMCSA procedures.

Alcohol Policy Acknowledgment

Indiana Dynamic Logistics LLC policy prohibits employees from using illegal drugs or abusing alcohol. An employee who refuses to submit to required drug and/or alcohol testing or tests positive may be subject to disqualification or disciplinary action, up to and including discharge, consistent with applicable law and policy.
